IMMUNIZATION INFORMATION

As of January 7, 2003 a new Florida Statute was adopted concerning immunizations for all full-time Florida university students.

If you live on campus it is required that you provide documentation for the following immunizations:

Meningitis
Hepatitis (B)
Measles, Mumps, Rubella (MMR)
If you are a full-time off campus student it is required to have on record the dates of your MMR.

This is a state requirement and is already in effect. Please assist the university by supplying the requested information to the student health center as quickly as possible. It is our desire that you have the maximum protection available from these diseases. If you disregard this notice you may be subject to fines up to $100.00.

If you would like information regarding Meningitis and Hepatitis B it is available for you in the nursing office. Also, if you decline to receive the vaccines you must come in and sign the waiver form.
